Whether you are expecting a new child, have recently moved to our area, or are looking for a new medical home for your children, we thank you for trusting us with your child's health. OCC Provides a comfortable, caring environment so our patients and their families can feel at ease. We welcome new pediatric and adolescent patients.

Please complete this list of new patient forms at your convenience. Questions about which forms you need? Please call our office and our courteous staff will assist you in preparing for your child's first visit.

If you're preparing for your child's first visit, you can do a few things to help expedite the appointment. Please provide your insurance cards, complete immunization records and medical records from your previous pediatrician when you arrive for your visit.

Attention Parents

Attention Parents/Guardians of Omaha Childrens Clinic Patients 

In an effort to protect your children and your identity, OCC has updated all pertinent paperwork for our patients. We realize that this may pose an inconvenience in that you will be asked to complete forms again. Please understand If the new forms are not completed in their entirety with all information complete and up to date, you will be entered into our system as a SELF-PAY patient and all charges for your visits will be due at time of service. 

Additionally, Through our contracts with all insurance companies we are contractually bound to collect your copay at the time of check in. If you do not have your copay for your appointment, your appointment will need to be rescheduled. Starting Immediately You will need to provide your insurance card, photo id and copay at each and every visit. We sincerely appreciate your cooperation in helping us to ensure the safety and security of your children and your personal information.


UPDATES TO OUR OFFICE MASK POLICY

Dear Omaha Childrens Clinic Patients and Families,

Our top priority is to provide the best possible care for your family and ensure a safe environment for every child who visits our office. With the end of the public health emergency, we have updated our office mask policy as follow:

If a patient has symptoms of a contagious illness, such as fever, cough, or sore throat, we require them to wear a mask during their visit. However, for patients without symptoms, we leave the decision of whether to wear a mask up to them and their families. We always have masks available for those who need them.
